GPD Case Management
The Grant and Per Diem Case Management Program provides case management services to formerly homeless veterans entering permanent housing who require additional case management support as they transition to community living. These services provide a bridge from placement to successful tenancy and long-term housing retention through skill development and growth. The program is based on the Housing First philosophy, with the goal of harm reduction and housing stability.
Services are available to veterans residing in the following counties:
- Bergen
- Essex
- Hunterdon
- Middlesex
- Morris
- Passaic
- Somerset
- Sussex
- Union
- Warren
